Transition of care for stroke patients: an integrative review

Objective: to analyze, based on scientific evidence, how the transition of care for people with stroke occurs after hospital discharge. Methods: integrative literature review, conducted in databases Scopus, Web Science, Pubmed/MEDLINE, Latin American and Caribbean Literature Health Sciences, Nursing Database The Cumulative Index Allied Literature. A total 21 articles were included final sample, which then descriptively analyzed. Results: factors related adequate included: organizational barriers health services involved care, importance information, coordination care. Conclusion: studies express limits experienced by individuals, family members professionals systems that are fragmented rule out possibility integral times transition. Contributions practice: study reveals necessary articulation those who provide this grievance their families, recognize points be improved practices minimize complications arising from failures transitions.
